Registration Form
I/my minor child will be participating in the Ride for Life fund raising event on August 4, 2012. I understand the ride is open to individuals, families, and all capable, interested, bicyclists. Riders must be 18 years old or be accompanied by a parent or guardian. The local Right to Life affiliate reserves the right to reject applicants from riding.
By signing this waiver I agree to obey the rules below:
Bicycle helmets are required to be worn at all times while riding.
No headphone devices are allowed while riding (i.e.: Ipods, Bluetooth phones, etc.)
All riding must be done during daylight hours.
Obey all state and local laws.
Be courteous while on the trail – do not impede traffic and become a statistic.
A chase vehicle will have basic repair material in it.
For your enjoyment, you and your bicycle should be in good condition. You should be able to make your own minor repairs and carry a replacement tube. Pace yourself to allow for food and rest stops and to arrive at our destination by 4:00 p.m. Lunch will be served at a designated area and time. Please be aware if you choose to leave early or later, lunch may not be available when you arrive.
Each rider must submit this registration form prior to the ride. Registration is open until the day of the ride. However, riders registered after August 1, 2012, may not received a Right to Life T-shirt, and may have to make their own return trip arrangements. Registration on August 4th, will begin at 8:00 a.m. and close at 9:00 a.m.
Cost: $10.00 per rider pre-registration up to August 1, 2012. $15.00 registration fee day of ride.
